Walk down any skincare aisle and "dark spots" is treated as one problem with one fix: a brightening serum. In reality, that phrase covers at least three distinct conditions — melasma, post-inflammatory hyperpigmentation (PIH) and sun spots — with different triggers, different behaviors and, crucially, different treatment rules. Get the identification wrong and you can waste a year on the wrong routine. In melasma's case, you can actively make things worse.
So before we talk ingredients, we do what a dermatologist does first: the differential.
One label, three different problems
All hyperpigmentation shares one mechanism at the bottom: melanocytes — the pigment-producing cells in the lower epidermis — receive a signal (UV light, inflammation, hormones) and respond by making extra melanin. That's normally protective. Hyperpigmentation is that protection gone local and excessive.
What separates the three conditions is which signal is driving the overproduction, and whether that signal is still switched on. A post-acne mark is the echo of an inflammation that has already ended. A sun spot is the accumulated receipt of decades of UV. Melasma is a live, ongoing condition whose triggers — hormones, UV, heat, even visible light — keep firing. That single difference explains almost everything about how each one should be treated.
Melasma: hormonal, heat-sensitive, chronic
Melasma shows up as soft-edged, symmetrical patches — not discrete dots — typically across the cheeks, forehead, upper lip and jawline, in a pattern sometimes called "the mask of pregnancy." It's far more common in women, tends to start between the 20s and 40s, and is driven by a combination of hormonal influence (pregnancy, oral contraceptives, hormone therapy), genetic predisposition and light exposure. People with light-brown to olive and medium-deep skin tones are affected most often.
Two things make melasma the odd one out. First, it responds to more than UV: visible light (including sunlight through windows) and possibly even heat appear able to provoke it, which is why it can flare in summer even under diligent sunscreen use, and why hot yoga, saunas and steamy kitchens are on many dermatologists' watch lists for melasma patients. The evidence on heat is not fully settled, but the pattern is reported often enough that most specialists advise caution.
Second — and this is the honesty most brightening marketing skips — melasma is currently considered a chronic, relapsing condition. It can be faded substantially, sometimes dramatically. It cannot, on present evidence, be permanently cured. Patches that clear over winter often return with the first sunny holiday. The realistic frame is management: a maintenance routine you keep, ideally built in partnership with a dermatologist, rather than a spot you erase once. That's not defeatism; people who accept this frame typically get much better long-term results than those who cycle through aggressive fixes and rebounds.
Post-acne marks: pigment left behind by inflammation
The flat brown, red or purplish marks that linger after a breakout are post-inflammatory hyperpigmentation. The mechanism is in the name: the inflammation from a pimple (or a burn, a bug bite, an over-enthusiastic peel) stimulates nearby melanocytes, which dump extra pigment into the healing skin. The result is discoloration without texture change — which matters, because a flat mark is PIH, while a dent or raised area is a true scar and needs entirely different treatment.
PIH deserves special attention in melanin-rich skin. The more reactive your melanocytes, the more pigment an identical pimple leaves behind — so in deeper skin tones, the mark frequently outlasts the acne itself and becomes the primary concern. This cuts two ways: treating active acne early and gently matters more (see our guide to choosing the right acne active, and the internal factors that drive breakouts), and aggressive treatments carry a higher risk of causing new PIH while removing the old. Deeper skin tones don't need harsher treatment; they need smarter, better-buffered treatment.
The good news: PIH is the most self-resolving of the three. The trigger has already ended, so with sun protection and time, most marks fade over months. The single biggest thing you control is not creating more of it — every squeezed or picked spot restarts the inflammation clock and deepens the deposit.
Sun spots: the slow stamp of cumulative UV
Sun spots — solar lentigines, sometimes miscalled "age spots" or "liver spots" (they have nothing to do with the liver) — are discrete, well-defined brown spots on the areas that have seen the most sun over a lifetime: face, backs of hands, shoulders, décolletage. They reflect cumulative UV exposure rather than hormones or a recent injury, which is why they typically appear from the 40s onward and multiply slowly.
Unlike melasma, lentigines are stable: they don't flare and relapse with hormonal shifts, and once treated (they respond comparatively well to professional options like lasers and cryotherapy, alongside topicals) they are less prone to rushing back — provided sun protection continues. One serious caveat: because a lentigo is a flat brown spot, it lives in the same visual neighborhood as early melanoma. Any spot that is new and growing, irregular, multi-colored, itching or bleeding is a doctor visit, not a serum decision.

Why the diagnosis changes the treatment
Here is where the differential stops being academic. The three conditions overlap on maybe 70% of their care — sun protection, pigment-inhibiting topicals, patience. The other 30% is where people get hurt:
- Melasma punishes aggression. This is the counterintuitive one. Strong peels, high-energy lasers and harsh scrubbing generate heat and inflammation — and inflammation is itself a pigment trigger in melasma-prone skin. A treatment that fades a sun spot beautifully can leave a melasma patch darker than it started. Melasma care is deliberately gentle, layered and boring.
- PIH rewards patience over firepower. Because the trigger has passed, the win condition is protect-and-wait, accelerated by mild actives. Over-exfoliating to "scrub off" marks re-inflames the skin and manufactures fresh PIH.
- Lentigines under-respond to serums alone. If your goal is erasing established sun spots, topicals lighten them somewhat but in-office options are the realistic endgame — while topicals and SPF prevent the next crop.
- The mimicry risk runs both ways. Melasma is regularly mistaken for sun spots (and treated too aggressively), and the occasional melanoma is mistaken for a harmless lentigo. When in doubt, the differential is a professional's job.
The ingredient arsenal, graded honestly
No single ingredient does everything. The well-supported ones either slow the pigment-production pathway (mostly by interfering with the enzyme tyrosinase), calm the signalling upstream, or speed the shedding of already-pigmented cells. Here's the field, graded on evidence rather than hype:

A realistic routine picks two or three of these — say, vitamin C by day, azelaic acid or a retinoid by night — over the whole shelf at once. Stacking five actives multiplies irritation, and irritation is a pigment trigger. Sunscreen: the non-negotiable (melasma wants it tinted)
Every path above runs through the same gate: daily sun protection. UV re-stimulates melanocytes faster than any serum can calm them, so skipping SPF turns months of treatment into running on a treadmill. This is true for all three conditions, without exception.
For melasma there's an extra, genuinely important detail: visible light — the ordinary bright light that ordinary chemical sunscreens do not block — can stimulate melasma, particularly in deeper skin tones. The practical fix is a tinted mineral sunscreen, because the iron oxides that create the tint physically block visible light in a way untinted formulas can't. If you have melasma and your sunscreen is untinted, this one swap is arguably the highest-value change you can make. The rest is unglamorous execution: broad-spectrum SPF 30–50 every morning, reapplication on high-exposure days, plus hats and shade during melasma flares. Boring, and more effective than any single active in this article.
Patchy, symmetrical and relapsing = melasma: treat gently, manage long-term, use tinted SPF. Flat marks where pimples were = PIH: protect, don't pick, let time and mild actives work. Discrete spots on sun-exposed skin after 40 = lentigines: SPF plus, realistically, in-office procedures.
When to see a dermatologist
Plenty of pigmentation is safely a skincare project. These situations are not — book the appointment when:
- Any dark spot is new and changing: growing, irregular in shape or color, itching or bleeding. Rule out melanoma first; treat cosmetics second.
- You suspect melasma — the diagnosis benefits from confirmation, and the strongest tools (prescription hydroquinone combinations, oral tranexamic acid, carefully chosen procedures) are prescription territory.
- Pigmentation appeared after starting a medication or alongside other symptoms.
- Six or more months of consistent SPF + topicals have produced no visible change.
- Your marks come with texture change — dents or raised areas are scars, and need procedural options topicals can't deliver.
- You have a deeper skin tone and are considering peels or laser — the right device and settings matter enormously for avoiding new PIH, so you want someone experienced with melanin-rich skin.

How do I know if it's melasma and not ordinary dark spots?
Pattern and behavior. Melasma forms soft-edged, roughly symmetrical patches on both cheeks, the forehead or upper lip, and it fluctuates — darker after sun or heat, sometimes shifting with pregnancy or contraceptive changes. Sun spots are discrete, stable dots; PIH sits exactly where blemishes were. If it's patchy, mirrored and moody, think melasma and get it confirmed by a professional.
How long does post-acne PIH take to fade?
Untreated but sun-protected, most marks fade over three months to two years — deeper skin tones and deeper deposits sit at the longer end. Consistent SPF plus ingredients like azelaic acid, niacinamide or a retinoid typically shortens this to a few months. Without sunscreen, the clock effectively keeps resetting.
What fades dark spots fastest?
For PIH and sun spots, the honest answer is a dermatologist's office: prescription combinations and procedures beat any serum for speed. In skincare terms, the fastest safe stack is daily SPF plus a retinoid at night and a tyrosinase-inhibiting active (azelaic acid, vitamin C or arbutin) by day. For melasma, "fastest" is the wrong goal — the aggressive shortcut is precisely what triggers rebounds.
Can melasma be cured permanently?
On current evidence, no — and any product promising a permanent cure is overreaching. Melasma is managed: it can fade dramatically with strict photoprotection and the right topicals, but the tendency remains, and flares follow sun, heat and hormonal shifts. The people with the best long-term skin treat it like an ongoing partnership with their dermatologist, not a one-off war.
